9/28/2023 EnchantaVaughn Approved Upon arrival to the school setting, the client appeared to be in a content state as evidenced by greeting QMHP-C with moderate emotions. During morning observations, the client exhibited difficulty following directives within three prompts while in his resource class. During midday observations, the client exhibited difficulty following directives within three prompts while completing his station work. During afternoon observations, the client exhibited difficulty following directives within three prompts and communicating with his teacher in an appropriate manner while in class. QMHP-C greeted the client upon arrival to the school setting. QMHP-C inquired about the clientâs morning. During morning observations, QMHP-C redirected the client and reiterated the importance of following directives within three prompts while in resource. During midday observations, QMHP-C redirected the client and reiterated the importance of following directives within three prompts while in his class. During afternoon observations, QMHP-C redirected the client and reiterated the importance of following directives within three prompts and communicating with his teacher in an appropriate manner. The client responded well towards Therapeutic Day Treatment services throughout the day as evidenced by participation and positive feedback. During morning observations, the client was receptive to redirection and agreed to follow directives within three prompts while in his resource class. During midday observations, the client was receptive to redirection and agreed to follow directives within three prompts while completing his station work. During afternoon observations, the client was unreceptive to redirection by using inappropriate language. The client was then able to calm down and agreed to follow directives within three prompts and to communicate with his teacher in an appropriate manner while in class. The client also agreed to continue to track his behavior on his self-monitoring chart.
